Job Objective:

Under the direction of the Director of Patient Care Coordination, or designee, the Manager Social Work is responsible for assisting Case Managers with direction and guidance pertaining to social work issues, as well as evaluating, assisting and developing an Integrated Case Management Patient Centered Plan of Care. The Manager Social Work is expected to function as a member of the interdisciplinary team regularly participating in the Acute Episode Life Cycle of Care across the care continuum.

Essential Duties and Responsibilities:

A. Social Work Case Management: Effectively plans, organizes and evaluates work duties of self and unit staff to promote quality social work, case management and patient care activities.

  1. Assist with the development of department goals ongoing, annually with the Director of Case Management in alignment with the hospital’s philosophy, goals, and objectives.

  2. Collaborates with internal as well as external providers to draft, implement and manage care directives in the Plan of Care.

  3. Responsible for assisting in the planning, scheduling and maintaining of Case Managers and Social Workers work schedules in a manner that incorporates continuity of care, staff competency and patient acuity.

  4. Develop, draft, and complete bio-psycho-social assessments.

  5. Create, organize, document, evaluate and implement treatment Patient Centered Plans of Care. Regularly evaluates patient care delivery of Social Workers from data compiled from written documentation, observation, and comparison to established standards.

  6. Participates in multidisciplinary rounds and ancillary rounds to provide a safe, clean and secure environment for patients, staff and visitors.

  7. Initiates, oversee, delivery culturally sensitive clinical interventions and short term counseling as determined by the interdisciplinary team or referral.

  8. Regular oversight/participation in the coordination of care planning of Social Workers in Discharge Planning and Post-Acute Care Planning in accordance Evidence Based Practices and Standards of Care for Social Worker.

  9. Participates in Transitions of Care. Promotes communication interdepartmental.

  10. Researches, communicates and recommends alternative cost effective health care services to the health care team. Participates in the Utilization Review of patient with complex bio-psycho-social needs.

  11. Directs and provides Crisis Intervention for acute patient as needed.

  12. Supports Social Workers in obtaining and providing community resources for patient and their families.

  13. Serves as a patient and hospital advocate regarding all clinical, social, financial and ethical healthcare matter.

  14. Performs/Oversight of Social Workers in the reporting of suspected child, domestic, adult and elder abuse or neglect.

  15. Assist with consultations for discharge planning.

  16. Performs/assist with the coordination of adoptions in accordance with hospital policy.

  17. Coordinates family conferences.

  18. Provides training/education to social workers as well as hospital staff, as requested. 

B. Builds and leads a high performing Team: Motivates, inspires and develops employees to accomplish goals and objectives.

  1. Interviews and selects staff for designated unit.

  2. Contributes to staff meeting at least monthly to discuss department and hospital goals, progress toward goals, and other significant information concerning the unit and organization in collaboration with the department director.

  3. Maintains staff meeting at least bi-monthly to discuss department and hospital goals, progress toward goals, and other significant information concerning the unit and organization in collaboration with the department director.

  4. Provides an environment that promotes employee satisfaction and retention.

  5. Empowers staff to participate in changing process and incorporate changes into daily practices. Recognizes employee contributions.

  6. Monitors employee performance and provides ongoing feedback. Initiates corrective action when indicated. Completes employee performance appraisals according to Human Resources guidelines.

  7. Empowers staff to take ownership of patient care environment.

  8. Avoids preventable turnover. Initiates plan for change as appropriate. Ensures completion and processing of all personnel forms.

  9. Participates in recruitment events.

C. Organizational Teamwork and Collaboration: Collaboratively works with peers and others to set and achieve interdepartmental / organizational goals and to resolve barriers to interdepartmental / organizational performance.

  1.  Works with peers and co-workers to resolve conflict and duplication of effort and waste.

  2. Communicates to clinical and ancillary staff information essential for continuity of patient care.

  • Attends required meetings or delegates responsibility when indicated in event of absence.

  2. Evaluates the educational needs of employees and provides for inservices/training monthly and as applicable
