MSW Social Work Case Manager - Case Management - Temporary/Full Time/Varied in Lancaster, CA at Antelope Valley Hospital

Job Description

Job Objective:

Under the direction of the Director of Patient Care Coordination, or designee (Manager), the Social Work Case Manager is responsible for evaluating, assisting and developing a discharge plan for patients and their families who have complex bio-psycho-social needs. The Social Work Case Manager is expected to function well as a member of the interdisciplinary team and participate in the referral development process. 

Essential Duties and Responsibilities:

A. Social Work Case Management

  1. Completes bio-psycho-social assessments, develops a treatment plan for referred patients and documents in the hospital medical record according to department policy.
  2. Assesses for Home Health and DME needs, makes referrals to the appropriate agencies and documents in the medical record to facilitate the completion of arrangements.
  3. Communicates the treatment and discharge plan to the healthcare team.
  4. Participates in multidisciplinary rounds and trauma rounds.
  5. Initiates culturally sensitive clinical interventions and short term counseling with input from the interdisciplinary team.
  6. Provides culturally sensitive education to enhance the patient and family's understanding of diagnosis, care plan, ability to cope with illness or injury and the continuum of care.
  7. Participates with patients, their family, the Case Manager and/or the Discharge Coordinator in Post Acute Care Planning involving complex bio-psycho-social needs.
  8. Facilitates transitions to alternate levels of care.
  9. Researches, communicates and recommends alternative cost effective health care services to the health care team.
  10. Provides Crisis Intervention.
  11. Educates patients and their family about resource options and how to access community resources.
  12. Serves as a patient and hospital advocate regarding all clinical, social, financial and ethical healthcare matters.
  13. Evaluates cases of suspected child, domestic adult, and elder abuse or neglect.
  14. Provides consultation to hospital staff and clinical oversight to ensure a safe discharge is arranged.
  15. Identifies and reports abuse of children and adults as mandated by state law.
  16. Coordinates and facilitates adoptions and surrogacy.
  17. Coordinates family conferences.
  18. Provides in-service training to hospital staff, as requested. 

B. NICU Social Work, As Assigned

  1. Participates in and attends NICU/discharge planning rounds with the Interdisciplinary Team.
  2. Attends pre-natal rounds/case finding.
  3. Completes a comprehensive psychosocial evaluation within 48 hours of admission.
  4. Maintains weekly summaries and completes discharge note as appropriate.
  5. Has working knowledge of CCS regulations.
  6. Maintains ongoing contact with the baby's family during NICU stay.
  7. Coordinates and attends physician conferences with families.
  8. Contributes to utilization management and the CCS referral process.
  9. Must be CCS paneled within 6 months of hire. 

C. Pediatric Social Work, As Assigned

  1. Assess CCS pediatric patients with LOS of 3 or more days.
  2. Maintain weekly summaries for CCS pediatric patients with LOS of 3 or more days and completes discharge note as appropriate.
  3. Must be CCS paneled within 6 months of hire. 

Non-Essential Duties:

  • Assist with other duties as assigned, within skill sets and abilities 

Knowledge. Skills and Abilities:

Knowledge

  • Working Knowledge of InterQual Guidelines
  • Working knowledge of Government, State, HMO, PPO, Commercial, and Workers Compensation utilization, authorization and billing guidelines
  • Working knowledge of Microsoft Office applications
  • Working knowledge of medical terminology 

Skills

  • Proficient in the operation of basic personal computers
  • Proficient in the operation of scanners, copiers, and fax machines
  • Good oral and written communication skills
  • Good interpersonal skills and customer focus

Education and Experience:

Education

  • Master in Social Work (MSW) from an accredited program by Council of Social Work Education.
  • Experience in a recognized area of Social Work
  • Previous Social Work experience in an acute care setting, preferred 

Required Licensure and/or Certifications:

  • It is required that eligible Social Workers maintain a current registration through the Board of Behavioral Sciences as an Associate Clinical Social Worker (ACSW) or applied for within six months after hiring.

Key Physical Requirements and Working Conditions:

  • Primarily works in a climate-controlled area
  • Standing and/or walking 75% of time on duty
  • Tolerate repetitive arm and hand movements
